SUPPORT THE PERSON
BEHIND THE CARE PLAN.
A change in living arrangements can bring changes in routines, relationships and a person’s sense of independence.
Our coaching gives residents space to talk about what matters to them and identify practical steps they want to take. Sessions can explore:
- Finding purpose and enjoyment in everyday life
- Building connection and supportive relationships
- Navigating change and managing everyday stress
- Developing achievable wellness routines within their care team’s guidance
- Recognizing progress and building confidence through small steps
Each participant brings their own priorities. We guide the conversation with respect for their choices, abilities and lived experience.
BRING COACHING
TO YOUR RESIDENTS.
Add a structured coaching program without having to develop and lead every session yourself.
We provide the coach, live online sessions and a clear program structure. Your community identifies interested residents and designates a staff contact to coordinate participation and any onsite support.
Simple ways to participate
Residents may join together in a suitable community room or individually using an appropriate device. We’ll discuss technology, accessibility and assistance needs before the program begins.
Remote delivery across the United States
Designed for communities nationwide, with suitability and any applicable requirements reviewed before launch.
START SMALL.
LEARN WHAT WORKS.
Discuss your community.
We meet with your leadership, wellness or resident services team to understand your goals and the residents you hope to serve.
Plan the group.
Together, we identify up to 10 suitable participants and agree on the schedule, session format and support needed.
Bring residents together.
We lead weekly live online coaching sessions focused on participant goals, practical steps and supportive discussion.
Review the experience.
At the end of the pilot, we review participation and feedback with your team, respecting resident consent and privacy. Then we discuss whether to continue or expand.
A PROGRAM THAT FITS
YOUR COMMUNITY.
Community sponsored coaching
Your organization purchases the program and we deliver the coaching through our existing infrastructure. This provides a way to offer the program without depending on insurance reimbursement.
Other private payment arrangements
We can discuss resident or family payment options if they fit your community’s policies and preferences.
Program scope and investment are discussed during your discovery call and outlined in a tailored proposal.
REQUEST A PARTNERSHIP CALLEXPLORE POTENTIAL
INSURANCE REIMBURSEMENT.
Reimbursement opportunities for health and wellness coaching are evolving.
If your community works with a physician or other eligible billing practitioner, we can discuss whether a clinically coordinated coaching arrangement warrants review by their billing team.
Verify the specific arrangement
Coverage must be verified for the specific service, resident, payer and care setting, including remote group delivery. Any required practitioner involvement, coach qualifications, documentation and billing arrangements must be established before relying on reimbursement.
A program with or without coverage
Insurance payment is not guaranteed. A community sponsored program remains an option when coverage is unavailable.
A coaching code alone does not make a facility or coach eligible to bill insurance.
Policy background: NBHWC coaching coverage update. Proposed changes must not be treated as confirmed coverage for this program.

QUESTIONS YOUR TEAM MAY HAVE
Is this medical care or therapy?
No. Coaching supports personal goals and behavior change. It does not replace medical care, mental health treatment, rehabilitation or your community’s existing services. Coaches do not diagnose, prescribe or change treatment plans.
Is this suitable for every resident?
Suitability is discussed with your team before enrollment. We consider each resident’s interest, ability to participate in a group conversation and support needs. Memory care or more complex needs require a separate discussion about whether this format is appropriate.
What does our staff need to do?
Your team helps coordinate participants, the meeting space or devices and any assistance residents need during sessions. We agree on these responsibilities before launching the pilot.
Can we participate without insurance coverage?
Yes. Your community can purchase the program directly. Other private payment arrangements can also be discussed.
Can we offer this at multiple locations?
Yes, we welcome conversations with organizations that operate multiple communities. A pilot at one location can help shape a broader rollout.
